On integrative biophysics in genito-urology on example of vesical detrusor and trigone (#413)

INTRODUCTION:Cardinal medical problems of humanity depend on genito-urology: contraception/incontinence/renal-hypertension,etc. whereby biophysical and phamaco-physiological research has a central position. New related to summarized earlier long-term observations (2014-1970) are given.

Methods: References [1-3, esp. for A:2d,e/1975,3a,b, B:1b,2a, C:2e/1991,2f,3b/1989, D:1a-c,2a, E:2g].

RESULTS: Significance p<0.05-0.01.

A. PROBANDS-PATIENTS(cystotonometry/CTM,n=150):Andrological-gynecological-urological patients have increased vesical-pressure/prolonged micturition/decreased urothelial-electropotentials after radiotherapy (radiocystitis Gamma-/X-irradiation-70Gy). Pharmaco-&adjuvant respiratory/music/yoga-therapies support recovery.

B. ANIMALS(rat/guinea-pig/GP,n=30):Isovolumetric CTM in vivo et in situ/in vitro et in toto: Urinary-bladder/UB generates spontaneous-phasic-contractions (SPC:3.46±1.17/min), after critical intravesical-pressure (>20mmHg) slow-tonic-contractions (STC:0.24±0.08/min). X-rays:50Gy induce vesical-tone-elevation.

C. ORGAN-PREPARATIONS(isometric rec.,n=420): SPC/STC appear in GP/rabbit/human-UB. GP-detrusor-SPC 4.04±0.75/min/trigone-STC 0.28±0.15/min after stretch (3-80mN). After irradiation (10-100Gy) vesical-tone increases, STC-frequency decreases.

D. DETRUSOR-MYOCYTES (intracellular rec.,n=120): After stretch (3-80mN) appear spikes-transformation (63.29±4.96/min) into burst-plateaus (1.54±1.18/min), augmentation-rate of spikes-rise (RR:0.41±0.19/3.27±0.76V/sec)&-fall (RF:0.43±0.28/2.32±0.58V/sec resp.).

E. NON-ADRENERGIC/-CHOLINERGIC DETRUSOR-NEUROTRANSMITTER (n=50): are evident via biological-assays (neurogenic contractions electrical-stimuation (10/100Hz,0.3ms,3s).

CONCLUSIONs:Biophysical-clarification of molecular-electromechanisms (A-E), correlation with mechano-sensitive ionic-channels, motor-activity-SPC/STC, non-identified neurotransmitters could open new-dimension for vesical electro-/pharmacotherapies: incontinence-nephrolithiasis-overactive bladder-radiocystitis,etc.
